    Narcisistici patologici e pericolosità socio-economica

    Il saggio intende fornire una personale rilettura degli studi sul narcisismo patologico in chiave socio-economica, ovvero avanzare l’ipotesi per cui i soggetti affetti dal suddetto disturbo di personalità (disturbo narcisistico della personalità “DNP”), possano poi essere più portati/tendenti a compiere atti di criminalità economica. L’analisi si basa sull’assunto empirico che il criminale economico gioca lo stesso ruolo nei confronti dell’organizzazione sociale che il narcisista assume nei confronti della vittima all’interno di una relazione. In entrambi i casi, tanto la societas che la persona sono “vittime invisibili” ed inconsapevoli del gioco al massacro che il disturbato sta intentando loro, impostato sulle loro debolezze. A tratti, paradossalmente, ne diventano anche complici, osannandone ed esaltandone le caratteristiche, e contribuendo inconsapevolmente a creare un “grande bluff” attorno alla loro figura. D’altra parte l’organizzazione socio-economica pubblica ha bisogno ed incoraggia la creazione di realtà imprenditoriali spregiudicate, dedite solo agli introiti, non capendo che però poi, alla fine, se fomenta questi valori all’eccesso, i danni che ne derivano sono incalcolabili, non sono per le singole persone ma per l’intera struttura socio-economica di base

    Estrategias para fortalecer la continuidad académica en la Licenciatura en Obstetricia (una “carrera de mujeres”)

    El propósito de esta comunicación es narrar la experiencia luego de la implementación de una cursada intensiva de verano de Biología Celular y Genética, del primer año de la Licenciatura en Obstetricia, de la Facultad de Ciencias Médicas, que ha brindado una nueva oportunidad para la aprobación de la materia. Del análisis de los datos se desprende que la cursada intensiva de verano ha contribuido al proceso de continuidad en este recorrido educativo-formativo, brindado una oportunidad diferente a las estudiantes que claramente ha sido exitosa dado el alto porcentaje de aprobación y promoción y un visible aumento de la retención de las estudiantes. Probablemente la época estival haya permitido a las estudiantes mujeres trabajadoras y mamás, entre otras variables, encontrar el tiempo para dedicarse más a su propia formación académica, delegando transitoriamente tareas domésticas como así también el cuidado y educación de sus hijos. Finalmente, a través de esta experiencia, hemos contribuido a fortalecer la continuidad en sus trayectorias educativas.Secretaría de Asuntos Académico

    Concise Review: Chronic Myeloid Leukemia: Stem Cell Niche and Response to Pharmacologic Treatment

    Nowadays, more than 90% of patients affected by chronic myeloid leukemia (CML) survive with a good quality of life, thanks to the clinical efficacy of tyrosine kinase inhibitors (TKIs). Nevertheless, point mutations of the ABL1 pocket occurring during treatment may reduce binding of TKIs, being responsible of about 20% of cases of resistance among CML patients. In addition, the presence of leukemic stem cells (LSCs) represents the most important event in leukemia progression related to TKI resistance. LSCs express stem cell markers, including active efflux pumps and genetic and epigenetic alterations together with deregulated cell signaling pathways involved in self-renewal, such as Wnt/β-catenin, Notch, and Hedgehog. Moreover, the interaction with the bone marrow microenvironment, also known as hematopoietic niche, may influence the phenotype of surrounding cells, which evade mechanisms controlling cell proliferation and are less sensitive or frankly resistant to TKIs. This Review focuses on the role of LSCs and stem cell niche in relation to response to pharmacological treatments. A literature search from PubMed database was performed until April 30, 2017, and it has been analyzed according to keywords such as chronic myeloid leukemia, stem cell, leukemic stem cells, hematopoietic niche, tyrosine kinase inhibitors, and drug resistance.     En nuestro recorrido, Arte, Ciencia y otros "subuniversos" se ponen en juego y se integran en la búsqueda por plantear al Chagas más allá de los abordajes tradicionales en contextos educativos diversos. En este trabajo iniciamos el proceso de sistematización del eje Arte-Chagas que atraviesa la propuesta del Grupo ¿De qué hablamos cuando hablamos de Chagas? (La Plata, Argentina). La inclusión del arte como lenguaje de expresión, comunicación y conocimiento quedó plasmada en una suerte de "Galería de Arte" conformada por ejemplos significativos que ilustran nuestro relato.     ANKRd44 gene silencing: a putative role in trastuzumab resistance in HER2-like breast cancer

    Trastuzumab is an effective therapeutic treatment for Her2-like breast cancer; despite this most of these tumors develop resistance to therapy due to specific gene mutations or alterations in gene expression. Understanding the mechanisms of resistance to Trastuzumab could be a useful tool in order to identify combinations of drugs that elude resistance and allow a better response for the treated patients. Twelve primary biopsies of Her2+/hormone receptor negative (ER-/PgR-) breast cancer patients were selected based on the specific response to neoadjuvant therapy with Trastuzumab and their whole exome was sequenced leading to the identification of 18 informative gene mutations that discriminate patients selectively based on response to treatment. Among these genes, we focused on the study of the ANKRD44 gene to understand its role in the mechanism of resistance to Trastuzumab. The ANKRD44 gene was silenced in Her2-like breast cancer cell line (BT474), obtaining a partially Trastuzumab-resistant breast cancer cell line that constitutively activates the NF-kb protein via the TAK1/AKT pathway. Following this activation an increase in the level of glycolysis in resistant cells is promoted, also confirmed by the up-regulation of the LDHB protein and by an increased TROP2 protein expression, found generally associated with aggressive tumors. These results allow us to consider the ANKRD44 gene as a potential gene involved in Trastuzumab resistance

    Prognosis of screen-detected breast cancers: results of a population based study

    BACKGROUND: The reduced mortality rate from breast carcinoma among women offered screening mammography is demonstrated after 15–20 years of follow-up. However, the assessment of 5-year overall and event-free survival could represent an earlier measure of the efficacy of mammography screening program (MSP). METHODS: All cases of breast cancer diagnosed in the Province of Modena between years 1996 and 2000 in women aged 50 to 69 years, were identified through the Modena Cancer Registry (MCR). Stage of disease and treatment information were obtained from clinical records. All the events occurring up to June 30, 2003 were retrieved by experienced monitors. Five-year overall and event-free survival were the principal end-points of the study. RESULTS: During a 5-year period, 587 primary breast cancers were detected by the MSP and 471 primary breast cancers were diagnosed out of the MSP. The screen-detected breast cancers were smaller, more likely node negative, with low histological grade, low proliferative activity and positive receptors status. Furthermore, the breast cancer diagnosed through the MSP more frequently received a conservative surgery. The 5-year survival rate was 94% in the screen-detected group, versus 84% in the other group (p = 0.0001). The rate of 5-year event-free survival was 89% and 75% for the MSP participants and not participants, respectively (p = 0.0001). CONCLUSIONS: Our data confirm a favourable outcome of screen-detected breast cancers in terms of five-year overall and event-free survival, which reflect the good quality assurance parameters of the MSP. Finally, a cancer registry should be implemented in every area covered by screening programs

    Efficacy of a new technique - INtubate-RECruit-SURfactant-Extubate - "IN-REC-SUR-E" - in preterm neonates with respiratory distress syndrome: Study protocol for a randomized controlled trial

    Background: Although beneficial in clinical practice, the INtubate-SURfactant-Extubate (IN-SUR-E) method is not successful in all preterm neonates with respiratory distress syndrome, with a reported failure rate ranging from 19 to 69 %. One of the possible mechanisms responsible for the unsuccessful IN-SUR-E method, requiring subsequent re-intubation and mechanical ventilation, is the inability of the preterm lung to achieve and maintain an "optimal" functional residual capacity. The importance of lung recruitment before surfactant administration has been demonstrated in animal studies showing that recruitment leads to a more homogeneous surfactant distribution within the lungs. Therefore, the aim of this study is to compare the application of a recruitment maneuver using the high-frequency oscillatory ventilation (HFOV) modality just before the surfactant administration followed by rapid extubation (INtubate-RECruit-SURfactant-Extubate: IN-REC-SUR-E) with IN-SUR-E alone in spontaneously breathing preterm infants requiring nasal continuous positive airway pressure (nCPAP) as initial respiratory support and reaching pre-defined CPAP failure criteria. Methods/design: In this study, 206 spontaneously breathing infants born at 24+0-27+6 weeks' gestation and failing nCPAP during the first 24 h of life, will be randomized to receive an HFOV recruitment maneuver (IN-REC-SUR-E) or no recruitment maneuver (IN-SUR-E) just prior to surfactant administration followed by prompt extubation. The primary outcome is the need for mechanical ventilation within the first 3 days of life. Infants in both groups will be considered to have reached the primary outcome when they are not extubated within 30 min after surfactant administration or when they meet the nCPAP failure criteria after extubation. Discussion: From all available data no definitive evidence exists about a positive effect of recruitment before surfactant instillation, but a rationale exists for testing the following hypothesis: a lung recruitment maneuver performed with a step-by-step Continuous Distending Pressure increase during High-Frequency Oscillatory Ventilation (and not with a sustained inflation) could have a positive effects in terms of improved surfactant distribution and consequent its major efficacy in preterm newborns with respiratory distress syndrome.
